Founded in 1991, Hope Solutions "formerly Contra Costa Interfaith Housing" is a vibrant and socially responsive non-profit agency that provides permanent housing and vital support services to over 1,500 families and individuals in Contra Costa County who are homeless or are at-risk for homelessness. Over 40 faith-based communities support our work, although there are no religious requirements or affiliations required to receive services.


Hope Solutions just completed a bold 5-year strategic plan that calls for the agency to triple the number of people we house and serve by 2024. A FEW OF US...


DeanneExecutive Director—lives in Central Contra Costa County with her husband and three children. She enjoys hiking the East Bay hills, camping, reading, cooking, and cheering too loudly at her kids’ games.


Alex—Chief Operating Officer lives in Walnut Creek with his wife, son, and dog.  Alex has an MSW and is a self-described macro social worker. He is an avid reader and a huge baseball fan (Go Giants!). He loves spending time at home with his family making food, listening to jazz, and relaxing.


Sara—Director of Programs—lives in West Contra Costa County with her partner and has raised four children. She enjoys walking, knitting, dancing, writing poetry, reading, and Burning Man. Sara is an LCSW and holds a Doctorate in Education.


Bill—Director of Housing Programs—lives in Central Contra Costa County with his partner and is involved in raising his godson. Bill is a licensed Marriage and Family Therapist who has been working with under-served populations for more than 20 years. He also serves on the Contra Costa County Healthcare for the Homeless Co-Applicant Board. He enjoys reading, gardening, and meditation. 


Christina—Director of Finance—lives in Pleasant Hill with her husband, three children, and their dog.  In her spare time, she volunteers at her children’s schools and with Girl Scouts, is on the PTA Board and local AYSO Board.  She also enjoys attending her children’s many sports activities.


Tanya—Director of Housing Navigation—lives in East Contra Costa County. She is married with one daughter and two sons. She has a Master's in Counseling Psychology and has worked with under-served populations for over 15 years. She enjoys music, comedy, movies, exercising, and watching basketball games.

 

Suzanne—Director of Development—lives in Alameda with her two teenage sons. She is a Rotarian, a member of the Alameda Elks Lodge, and serves on the Board of Directors for the Golden Gate Area Council Boy Scouts of America. She enjoys all things nature, including wilderness camping, off-road exploring, kayaking, and wandering the diverse terrain of California.


Kristina—Director of Human Resources—lives in Cordelia with her modern-day blended family and is a proud Air Force Mom. She enjoys spending time with family, yoga, indoor cycling, hiking, brunch with friends, and loves to laugh. She believes if you look for the good in all things, you will find it.

 

WHO WE ARE LOOKING FOR:

 

Hope Solutions seeks an energetic Housing Advocate who is flexible and responsive to support the needs of our clients while possessing the skill set to collaborate with interdisciplinary team members and community partners. We are looking for compassionate service-driven individuals who fundamentally believe in the potential of others. The Mental Health Intern is responsible for providing on-site mental health and case management support to children and their families living at Garden Park and for adults, youth, and families who live in scattered-site permanent supportive housing.  

 

Supervisory Responsibility:  None


WHAT YOU WILL DO:


Mental Health Support Services – Garden Park Apartments (GPA):

  • Provide milieu-based therapeutic services to individual children living at Garden Park Apartments (GPA) who are experiencing emotional challenges. Many of these children may be receiving EPSDT services.
  • Provide on-site individual and group therapeutic services to children at GPA.
  • Assist in delivering on-site parenting support groups to families at GPA.
  • Complete accurate and timely EPSDT chart notes for all services provided to children who are assigned under this contract.
  • Meet individual EPSDT contract goals for hourly billing, including averaging a minimum of 7-12 hours per week (depending on work schedule) of direct service to clients who are assigned under this contract.
  • Collaborate with case managers and youth enrichment coordinator to provide support for families, using a team approach.                                                                              
  • Participate in weekly group and individual supervision, using that time to work on developing therapeutic skills and clinical knowledge, as well as exploring growth opportunities as a mental health provider. Participate in clinical training as possible/needed.


Mental Health Support Services – Scattered Site Supportive Housing Program (SSSHP):

  • Provide individual and group case management and therapeutic services to formerly chronically homeless adults experiencing mental illness who are living independently in the community.
  • Assist residents in developing individual life plans and goals – and provide support to assist them in successfully achieving these goals.
  • Provide crisis intervention to residents experiencing mental health crises, ensuring that they remain safe and supporting their ability to continue living independently, if possible.
  • Maintain case notes and required documentation in compliance with HIPAA regulations.
  • Collaborate with case managers to support residents with a team approach.
  • Participate in weekly group or individual supervision, using that time to work on developing therapeutic relationships and clinical issues, as well as exploring growth opportunities as a mental health provider. Participate in clinical training as possible/needed.

 

Mental Health Support Services – Families in Supportive Housing (FISH):

  • Provide in-home case management and therapeutic services to formerly chronically homeless youth and parents experiencing mental illness who are living in the community.
  • Assist residents in developing individual life plans and youth to develop academic and life skill goals – and provide support to assist them in successfully achieving these goals.
  • Provide crisis intervention to residents experiencing mental health crises, ensuring that they remain safe and supporting their ability to keep their families united and remain living independently, if possible.
  • Provide conflict mediation to residents at odds with family members or neighbors.
  • Maintain case notes and required documentation in compliance with HIPAA regulations.
  • Collaborate with case managers to support families with a team approach.
  • Participate in weekly group or individual supervision, using that time to work on developing therapeutic relationships and clinical issues, as well as exploring growth opportunities as a mental health provider. Participate in clinical training as possible/needed.

 

Depending on the program in which the intern works and the number of hours employed – additional responsibilities may include: 

Reports and Data Collection:

  • Assist the services team with the following data entry, reports, and forms:
    • Reports that document service delivery and progress towards goals for Hope Solutions programs.
    • Daily case notes for individual, family, and group work.
    • Daily notes for EPSDT clients.
    • EPSDT internal audits and filing of notes.

 

Program Enrichment Activities:

  • Assist with staff supervision of volunteer programs and activities delivered to families served by Hope Solutions. These may include weekend and evening commitments.
  • Work with other staff members to provide community resources for families and individuals, such as furnishings for new apartments, social and health-related activities, life skills and employment support, and youth enrichment.

  

Organizational/Administrative Responsibilities:

  • Professionally represent Hope Solutions in all circumstances.
  • Ability to establish and maintain professional boundaries in working with clients.
  • Comply with all local, state, and federal regulations, policies, procedures, standards of practice, and outcome requirements of the Supportive Housing Program and Hope Solutions.
  • Assist with distributing client satisfaction/feedback surveys for all clients served.
  • Attend all required meetings, including but not limited to Hope Solutions staff, individual supervision, consumer case conferences, and linkage meetings with other agencies.
  • Comply with all legal/ethical professional guidelines for maintaining consumer confidentiality, protecting consumer rights, advocating on consumers’ behalf, and assuring consumer safety.
  • Complete required administrative paperwork and reports in a timely and accurate manner.
  • Perform all other duties and responsibilities as assigned.
  • Maintain confidential data files that adhere to HIPAA regulations.
  • On-call, after-hours, and backup work for other team members may be required.


Position Qualifications:

  • Passion for and demonstrated experience successfully supporting vulnerable populations, especially homeless families with mental disabilities. (Mental health or ex-mental health consumers are encouraged to apply).
  • This position will require a highly organized individual with excellent follow-up skills as well as the ability to overcome objections and obstacles in the pursuit of success.
  • Demonstrates a positive attitude, self-motivation, organization, and resourcefulness while maintaining a reputation through proven action of being client-oriented.
  • Master’s degree in Social Work or Marriage and Family Therapy.
  • Registered with the Board of Behavioral Sciences as an intern working toward licensure as an LCSW or LMFT.
  • Excellent written and verbal skills in the English language. Bilingual English Spanish is a plus.
  • Computer proficiency in the use of Microsoft and database applications.
  • Must have operational vehicle, auto insurance, valid California driver’s license, and be willing to drive for work.
  • Must pass LiveScan screening and TB test.
  • Salary contingent upon skill and experience.


Physical Qualifications:

  • The person in this position may make home visits to residents who live on second or third-floor apartments (without elevators) and will need to be able to climb stairs.
  • May accompany residents on walks to local resources (grocery store, childcare, parks) up to ½ mile in distance.
  • Must be able to work in a desk setting for up to 3 hours to make calls to residents, research resources on the internet, collaborate with community providers with calls/emails.
  • Office setting activities are required, such as typing on a laptop/computer to respond to emails, enter case notes into a database, copy and fax forms and documents on a copy machine related to maintaining or obtaining housing.
  • Occasionally must lift and carry objects weighing up to 25 pounds, including furnishings for new residents, files, holiday gifts, and food donations.
  • Ability to drive an automobile and transport oneself and residents to appointments or to meet at resident homes, including driving at night.
